Students Entering:
Kindergarten - need to have Updated Shots & Physical before enrolling in school in the fall. Also a simple Eye Exam needs to be completed either by Doctor during physical or at an Optometrist.
5-8th Grade - Students who plays sports on a St. Paul Lutheran Ministries team, need to have a Sports Physical completed and sent to school office, before attending practice.
7th Grade - Need to have Updated Shots & Physical. Even if not playing sports, as Physical needs to on file at school.
